Complaints naming a driverless vehicle barely existed before last year and then arrived in volume. Some of that is more vehicles on more streets. Some of it is that the federal complaint form only recently gained a field for naming the company operating the vehicle, so a complaint that would once have been filed against a Jaguar can now be filed against Waymo. Read the shape as the record catching up with the road as much as the road changing.

They are complaining about two completely different things

The distinction that decides whether these counts can be added.

Not one of the 33 complaints about a driverless vehicle describes a crash. They describe conduct: a vehicle stopping in a live lane, turning across somebody, blocking a crossing, sitting through a green light, or in one case carrying a small child with nobody at the wheel. That is what it looks like when the public reports a machine rather than a defect.

The 162 complaints about driver-assistance systems are the opposite: 39 of them describe a crash and 27 an injury. Those are owners of ordinary cars reporting that the system they switched on did something they did not expect. Both sit under the same federal component heading, and adding them together would produce a number that describes neither.

One city keeps its own count, and it is 28 times larger

San Francisco's transport agency runs a 311 category specifically for autonomous vehicles. It measures something the federal channel does not: not a defect, but a vehicle being a nuisance on a named street at a known minute. The bar to filing is a phone call rather than a federal complaint form, so the volume is a different order entirely — and it covers one city.

The system mistakenly determined that the vehicle was on the lower roadway, causing the brakes to apply erroneously. The contact stated that the failure was caused by Volkswagen's Predictive Adaptive Cruise Control. During the failure, the warning lights illuminated on the instrument panel and the message curve ahead, reduce speed was displayed. The manufacturer was notified of the failure by phone and email. The vehicle was taken to a local dealer, but the issue was deemed not to be a safety concern by the service department.

Waymo was contacted about the issue but declined to provide the contact with the year, make, model, or license plate information. Instead of executing the drop-off protocol, the automated vehicle entered an infinite loop, driving in continuous circles around the property over and over again. My son has a disability and became highly distressed and panicked was crying and very scared while contained inside.While the vehicle was actively malfunctioning, I called to Zoox Customer Support from inside the cabin. The remote human agent explicitly stated over the phone: "Yes, we can see exactly what is going on with the ride." Despite this live confirmation that the vehicle was trapped in a routing glitch and me and my son were in severe distress, the remote monitoring team failed to immediately override the system, shut down the loop, or pull the vehicle over to allow a safe exit.The vehicle ultimately logged a total trip distance of 6.15 miles and a 30-minute duration (Drop-off delayed until 1:06 AM) for a standard, direct route that should have been approximately 2.3 miles and under 10 minutes. Autonomous Vehicle Safety Complaint My family ordered a Waymo from the Phoenix airport. There were five passengers, including two children and two elderly adults, as well as luggage. One passenger sat in the driver’s seat. The vehicle accepted us and began driving without providing any warning that we were violating a passenger or seating policy. Several minutes into the trip, Waymo detected the violation and pulled over in an isolated parking lot with no safe pedestrian access to the airport or another pickup location. We were told to exit and arrange alternate transportation. However, Uber/Lyft/Waymo could not pick us up from that location. Despite approximately 110°F heat, two children, two elderly passengers, and our luggage, Waymo support refused our repeated requests to simply return us to the airport or move us to a safe pickup location because we had violated their policy. I ultimately had to walk to a nearby police station, where a police officer drove me back to the airport to get help. I understand Waymo’s passenger policies, but the vehicle should never have begun the trip if it detected too many passengers or someone in the driver’s seat. More importantly, passengers should never be stranded in an unsafe or inaccessible location because of a policy violation. I am reporting this because I believe Waymo needs safeguards requiring the vehicle to either refuse to begin the trip or, if a violation is discovered after departure, transport passengers to the nearest safe and accessible location. This incident created an unnecessary and potentially dangerous situation for our family.

Seconds later the contact's daughter attempted to disengage the self-driving feature and then depressed the brake pedal to stop from entering a right-turn-only minor road stop-controlled intersection. However, the front passenger’s side of the vehicle was struck by another vehicle, before striking another vehicle. The contact stated that the air bags on the passenger’s side of the vehicle deployed. The contact's daughter sustained minor leg injuries. Medical attention was not needed, and the contact was not sure whether occupants of the other vehicle required medical attention. A Police report was filed. The contact stated that the vehicle had not detected the oncoming traffic and that the crash could have been prevented. The vehicle was towed to a tow yard where it was deemed a total loss. On XXX, I was the sole passenger in a driverless Waymo (XXX) in [XXX]. Pickup: Chase Field ~4:14 PM. At ~4:25 PM on [XXX], a multi-lane urban arterial, the automated driving system decelerated the vehicle without cause to ~10 mph and held that speed in the number one (leftmost) lane. There was no obstacle, pedestrian, emergency vehicle, work zone, signal, or traffic condition to explain it. Traffic passed at normal speed in adjacent lanes. The system merged to the center lane, then returned to the number one lane, still at ~10 mph. The vehicle was a slow-moving obstruction in a live travel lane. There was no steering wheel, no pedal, no human driver. I could not accelerate, pull over, or exit the lane. My exposure to a rear-end collision was created solely by the ADS and could be ended only by the ADS, which was not ending it. I called Waymo rider support. The vehicle remained at ~10 mph. About 30 seconds in, a remote agent overrode the system and restored normal speed. Key finding: the malfunction did not self-correct. The vehicle did not detect the anomaly, resolve it, or degrade to a safe state. It operated hazardously until a remote human took control. The system’s own safety architecture did not catch it. A human did, only because I called. Waymo confirmed in writing that it verified the event as described, then declined to provide root cause, telemetry, or any investigative outcome absent legal process. I request NHTSA determine: (1) whether this was reported under the Standing General Order for ADS vehicles, or deemed below threshold and on what basis; (2) whether unexplained deceleration, sustained low-speed travel in live lanes, or failure-to-self-correct events have been reported for this fleet in the Phoenix ODD; (3) whether an ADS that cannot correct its own hazardous speed loss without remote human rescue meets any reasonable safety expectation for driverless operation on public arterials. After disengaging Full Self-Driving (FSD), a mandatory questionnaire pop-up now stays on screen until manually answered, instead of auto-dismissing after a few seconds as it previously did. This forces the driver to interact with the touchscreen immediately after taking back manual control — a moment when full attention should be on the road, not a pop-up. The prompt blocks part of the instrument/display screen until it is tapped, and in some cases requires a minimum 5-second voice response to dismiss (a previous workaround allowing a near-instant voice note to clear it was removed). The pop-up has also been observed persisting even after the vehicle is shifted into Park. This creates a distraction risk during an already safety-critical moment (immediately following a system disengagement, which by definition means FSD failed to handle the driving task). This behavior has persisted across at least 3 software updates. Multiple attempts to raise this with Tesla directly have not resulted in a fix or response. Other FSD users have reported the same complaint. No collision has occurred as a result of this specific issue, to my knowledge, but the design creates a foreseeable distraction hazard during takeover events, which are often already high-risk moments.

The reporting citizen was driving legally on [XXX] going forward towards the parking lot next to the round about when an uncrewed, autonomous commercial vehicle operated by Waymo suffered a catastrophic software/hardware failure. The 6,000-pound driverless robot taxi aggressively pursued and chased the citizen’s vehicle in reverse for that full city block despite honking the horn and then holding it down the entire time as the killing 6,000 lbs robot car kept reversing towards the citizens car instead of going forward, forcing the citizen out into a perpendicular intersecting road onto [XXX] The citizen safely evaded a severe collision solely through high-speed defensive driving, reversing, and holding down the horn to warn surrounding motorists of the out-of-control vehicle. The autonomous system failed to stop, recognize the citizen's vehicle, or register the horn. Upon reviewing the internal vehicle data and footage, Waymo’s corporate office confirmed to the citizen on several calls after the [XXX] reporting that this specific vehicle "went rogue." Waymo admitted the failure was severe enough that they immediately pulled the specific vehicle out of operational service. I have several case #s from Waymo of each call admitting fault. These 6000 lbs killing car robots are NOT SAFE and should not be allowed on public roads where real humans are driving! I can’t sleep since [XXX] due to the nightmares of these killing car robots trying to kill me. I must factor in extra time to go anywhere as I cannot have these killing car robots near me and they are everywhere in my area. I visibly shake and sweat involuntarily whenever I see one of these even from very far away or even think about them—which is pretty much always now since [XXX]. On [XXX] at approximately [XXX] pm, I was driving in the right-most lane on [XXX]. Traffic in my lane was stopped due to a red light. An autonomous Waymo vehicle was originally stopped at a yield sign on an intersecting side road behind my position. Instead of waiting for a legal gap in the traffic lane, the Waymo vehicle illegally entered the designated bicycle lane to bypass the stopped vehicles. When the traffic light turned green and my lane began to move, the Waymo vehicle abruptly merged into my lane directly from the bike lane (my right-side blind spot). It forced its way in front of my vehicle, requiring me to brake and use my horn to avoid a collision. The autonomous vehicle performed an unpredictable, illegal passing maneuver in a non-vehicle lane (bike lane) and merged from a blind spot, causing a near-miss with a human-driven vehicle that had the right-of-way. The vehicle was traveling on a roadway with a speed limit of approximately 45 mph when it suddenly decelerated and continued driving at approximately 10 mph. Other vehicles on the road were traveling at normal traffic speeds, creating a significant speed differential. I was concerned that a vehicle approaching from behind could have rear-ended the Waymo vehicle due to the unexpected and substantial reduction in speed. The vehicle remained at approximately 10 mph for a period of time while surrounding traffic was moving significantly faster. There were no warning messages, alarms, or explanations provided to me before the deceleration. There was no collision, injury, or property damage, but I believe the situation created an unreasonable safety risk. I contacted Waymo support during the incident. I was informed that the vehicle had entered a "restricted zone," but I was not provided with a clear explanation of what that meant or why it required the vehicle to travel at approximately 10 mph on a roadway where traffic was moving much faster. No emergency services responded. There was no collision. However, I am reporting this incident because I believe the vehicle's behavior created a potential traffic hazard and should be reviewed from a safety perspective.

At 11:45 AM, a Waymo AV stopped in the middle northbound lane at the Kierland Commons/Scottsdale Quarter signaled pedestrian crosswalk. The stop signal was not on. Pedestrians were standing on the sidewalk waiting for the signal to turn so they could cross Scottsdale Road. I assume the Waymo AV saw the pedestrians standing on the sidewalk and decided to stop. I was directly behind the Waymo AV. Traffic was passing us in both lanes. The Waymo AV remained stopped until the pedestrian signal came on and did not move forward until after the signal turned off. Fortunately, vehicles behind me were paying attention and did not rear-end anybody. The main issue involves a Waymo AV coming to a stop when it sees pedestrians waiting to cross the street, rather than paying attention to the pedestrian signal light, which is a blinking red light over each lane when triggered by a pedestrian that turns solid red, allowing pedestrians to cross the street. I have provided this information to Waymo LLC and they have responded that they are looking into why this Waymo AV stopped when it should not have stopped.

Somebody may have already submitted a blurb about their Tesla Y traveling on a highway with the speed limit of 60 giving an extra foot of room to a motorcyclist that was passing their vehicle. This happened to me today for the first time while I was in full self driving mode . My car was aware of the motorcyclist wanting to pass me and the car in the next lane.  Sensing this it moved over about a foot in my lane to allow the motorcycle passage  more room. A few minutes later, it did it again for a different motorcyclist. It did this all while in full self driving mode. Kudos to this safety technology.

A waymo put pedestrian and public safety at risk on 5/12/26 at approximately 7:50 pm when it stopped halfway through an intersection at 14th, Market, and Church Street in San Francisco alongside a crosswalk. I began honking from my vehicle as the waymo stopped in front of lanes of oncoming traffic, blocking the intersection. Instead of continuing through the intersection, the waymo turned towards oncoming traffic and advanced forward into the crosswalk and into oncoming traffic that was stopped at the red light. As one of the vehicles it was advancing towards I laid on my horn to try to get it to stop, but it didn't and the car next to me had to back up away from it to avoid collision. The waymo stopped about 1 foot away from the car before reversing into the intersection again (see pictures) and then turning onto 14th andriving away. The waymo essentially used a very busy intersection and active crosswalk to execute a 3 point turn. Without human driver intervention a collision was likely with a vehicle or pedestrian.

It was also near an intersection and if things were busy this might have caused an accident. It was the smart and calm defensive driving of multiple people and the truck driver that prevented an accident. It shouldn’t have to require that and my city streets shouldn’t be used as an experiment using vehicles that could be deadly. The transition from a left-turn lane into a straight path happened quickly, leaving extremely limited reaction time (estimated less than one second). As a result of the system’s unexpected behavior, a collision occurred with another vehicle traveling straight. At the time of the incident: Weather conditions were clear. Road markings were visible. There were no obstructions blocking lane markings. I was attentive and looking at the road. The police report assigned fault to me as the driver; however, the vehicle was actively operating under Tesla’s FSD system when it disregarded the clearly marked left-turn-only lane. They did not operate according to California Vehicle Code §21800 (d)(1), which states “The driver of any vehicle approaching an intersection which has official traffic control signals that are inoperative shall stop at the intersection and proceed when it is safe to do so.” The robotaxis did not proceed when traffic cleared and, in some reported instances, blocked a traffic lane for several hours. Waymo subsequently claimed to remedy this fault, stating online that “...we are now implementing fleet-wide updates that provide the Driver with specific power outage context, allowing it to navigate more decisively.” Waymo statement: [XXX] On December 29, 2025, a video from [XXX] was downloaded. Waymo ran a red light at [XXX] on [XXX] in Santa Monica, CA. It was traveling probably 20-25 mph (less than the speed limit) and could have easily stopped after the light turned yellow at [XXX] . Instead it continued through the intersection while the light turned red. I was a pedestrian waiting to cross and was delayed by the Waymo. I could have been hit if I entered with the walk signal. Please see video here: [XXX] . I reported this to Waymo. They refused to acknowledge the mistake or explain what specific actions they were taking in response. While this situation was not exceptionally dangerous, that was partly because I took caution. A human driver may slightly misjudge this sort of situation, but I cannot understand why an autonomous vehicle would do it. It was not a challenging driving situation for a computer to solve if it is generally trying to operate within the rules. I am concerned that Waymo is cheating a bit on red lights on purpose to save time. I strongly support autonomous vehicles for safety, but they only help if companies take the rules seriously. I am writing to raise a safety concern regarding the use of Tesla’s Full Self-Driving (FSD) system on my 2023 Tesla Model Y when a rear hitch-mounted bike rack is installed. Numerous users, including myself, have experienced dangerous or erratic behavior from FSD when carrying bicycles on a rear-mounted rack. Specific issues include: False detection of a trailing vehicle directly behind the car. Phantom braking or swerving, including abrupt lane changes or acceleration. Unreliable navigation at intersections (e.g., nearly running stop signs) due to apparent misinterpretation of sensor data. In some cases, tailgating or aggressive following behavior by the vehicle. These behaviors appear to stem from the FSD system misinterpreting the presence or visual signature of the rack and bikes as another vehicle. In several online reports, this has led to unsafe maneuvers. To temporarily mitigate the issue, some drivers have resorted to taping over the rear camera, which is neither safe nor user-friendly. While I recognize Tesla has made improvements such as camera obstruction warnings (e.g., in FSD v12.5.6+), these do not address the core misclassification problem. My requests: That Tesla officially acknowledge and investigate FSD misbehavior in the presence of rear-mounted bike racks. That Tesla provide clear driver guidance on whether FSD should be used when such a rack is installed. That the FSD system include an option for a “Bike Rack Mode” or alternative logic to prevent rear-camera misinterpretation. That this concern be escalated within your safety and AI development teams to ensure user safety is prioritized.

I am writing to report a concerning incident involving the Full Self-Driving (FSD) feature on my 2026 Tesla Model Y. My wife and I were traveling from Safety Harbor, FL to Foxtail Coffee on [XXX] , a trip of approximately two hours. The FSD system performed well for 98% of the journey. Suddenly, without warning, the car accelerated and ran the red light. Our 2022 Tesla Model Y Performance backed out of a driveway into the street and parked in the middle of the street with nobody inside the car, creating a traffic hazard. This happened after around an hour parked. The Smart Summon feature was enabled in the car. Possible cause was that Summon was accidentally activated by phone jostling in pants pocket. Car was looked at by Tesla and found to be in good working order and was considered safe to drive. No explanation of cause of the event was offered by Tesla. We turned off the smart summon feature in the car. Suggest adding an additional step in the Tesla app such as requiring a numerical code input to eliminate the possibility of accidentally summoning the car by random inputs to Tesla app when phone is in a pocket. There was one Zoox ahead of me and one behind in the left lane and I tried to merge in between with blinkers on. The Zoox car behind sped up to close the gap to block me out. After the stop light turned green zi tried to merge again before my lane ended and the driver did the same thing by closing the cap and almost hitting the Zoox ahead of him. It also looks like that driver intentionally partially blocked the first left turn lane so I couldn't use it. I have everything recorded on my Tesla Front, side and back cameras. I attached the footage in the email for review. We should all be concerned if the autonomous vehicle drives this way or if it's learning from this driver's driving behavior. I wanted to add that I had my wife and infant in the car. There's a baby on board sticker on the back, driver side so the Zoox driver had to have seen it and still decided to run me into the cones unless I stopped despite I was already on the dashed lines merging in. However, since the alterations, there is now an alarming 80% chance that, when engaging the Autopilot, an immediate alert demanding attention to the road is triggered. This issue is especially vexing when I am simply deactivating/reactivating the system for routine lane changes. The primary purpose of advanced driver-assistance systems, such as Tesla's Autopilot, is to enhance safety and provide a more efficient driving experience. However, the recent modifications seem to have had the opposite effect, introducing a distracting and potentially hazardous element that was not present in the original design. I understand the necessity of regulatory oversight, but it is crucial to ensure that the implementation of such changes does not inadvertently compromise the safety of the vehicles in question. As a concerned Tesla owner, I urge the NHTSA to thoroughly review the impact of the enforced alterations and consider the unintended consequences on the safety and performance of the Autopilot system. At around 3:30pm on 4/4/2022 I was driving northbound on 3rd Ave. SW (which is a one way) approaching W. Center Street. I was in the right lane, and one of the autonomous shuttle buses was in the lane next to me, on my left. As we neared the intersection with W. Center Street, while it was still next to my car, the shuttle began to turn into my lane. The shuttle buses have a maximum speed of only 15 mph so I was able to accelerate a little and get out of its way, so it wasn't all that dangerous and there was no contact. I thought it should be mentioned anyway, hopefully this can be used as friendly feedback to help them fine-tune safe operation of the shuttle buses.

Lane keeping assistance over corrected and caused an accident that resulted in total loss of my vehicle. My tire crossed white line , I was trying to gradually adjust to keep from over correcting. The lane keeping assistance caused the steering to pull in the opposite direction and would not allow me to correct. The more I tried to get back in the lane the harder the lane assistance pulled in the oppposite direction. It ultimatley jerked the steeing wheel in the opposite direction a caused me to hit a tree head on.

On trip from Grand Island NE to Pueblo CO on Colorado state Hwy 76 when driving at highway speeds the auto steer function repeatedly (at least 8 occurrences) invoked emergency braking for no apparent reason. This occurred during daylight hours, in brilliant sunshine, on clear and dry road conditions with no other vehicles in the vicinity, in both the right and left lane. In some cases the car would brake upon cresting a slight rise where the horizon is not visible until the vehicle is at the top of the rise. In another case there was highly reflective silver steel guard rails on both sides of the road. Also experienced phantom braking when there was identical signage on both sides of the road at about head height of a person. We also experienced occurrences on flat sections of road with no signage. The braking is unpredictable and severe enough to cause a rear end collision If a vehicle had been behind us during any of the events we experienced.

I was driving on a two lane road using auto pilot. I was going 55mph when all of a sudden my car slammed on the brakes down to 32mph as a school bus approached us. There was no reason to brake. The school bus never entered my lane. If a car had been following me closely I would have been rear ended for a system malfunction. Very scary.

I want to know when it became ok for Tesla to involve me, my wife, my kids, and other unsuspecting drivers in their beta test of their fully automated driving? I read an article in the Washington Post today about a Tesla recall due to cars in Fully Automatic mode rolling through stop-signs and I couldn't believe my eyes. I'M ON THESE ROADS, how does Musk get away with testing his software on any road that doesn't include ONLY people who have signed on to be a part of the test? I don't agree to participate in this beta test and I don't think it's fair for Tesla to test their software out on unsuspecting people. There was a commenter on the WaPo article today who said his car, in "Beta Mode" tried to pass a stopped school-bus with the lights on. What the hell makes this ok on public streets? The contact owns a 2020 Tesla Model 3. The contact stated that while driving on the right side of the road at 35-45 MPH, on autopilot mode with a passenger in the front passenger seat, the vehicle drove off the road independently and crashed into a tree. The air bags failed to deploy. There were no warning lights illuminated. The contact's face hit the steering wheel. The contact sustained a black eye with laceration, bruised ribs, head, neck and back pain, sprained left ankle, bruises on her thigh, may have sustained nerve damage on the right side of the face that hit the steering wheel and sciatic nerve damage on the right side of her body. The occupant of the front passenger seat sustained neck, back and clavicle injuries. The contact sought medical assistance the next day. The vehicle was towed to a tow lot, then it was towed to a Tesla certified specialist body shop. A police report was filed. The contact has not been able to get a hold of the manufacturer. The vehicle was not diagnosed or repaired. The failure mileage was 55,000.

On November 30 at 6pm I was driving north on I-75 in the Tampa area with Navigate on Autopilot activated. I was in the middle lane, it was dark, and there was lots of traffic. My screen asked me to confirm the autopilot request to move to the left lane. I tapped the stalk to confirm. I did not look in the left lane and was not controlling the steering, though my hands were on the wheel. The car immediately turned into the left lane and side swiped a police vehicle. Nobody was injured and the damage to both cars was minor. Previously I had received alerts that my left pillar camera was "blocked", but the alerts went away, so I assumed that there was no problem. When the alerts appeared, auto steer was deactivated, but when the alerts went away auto steer worked again. I do not remember when the first alert appeared, but probably earlier in November 2021. Subsequently, the alerts have appeared and disappeared. I have requested service from Tesla, but the appointment is scheduled for next month. The officer in the police vehicle and two other officers who came to the scene looked at my vehicle and took pictures. I received accident report, Crash Number [XXX] from Hillsborough County Sheriff's Office (Case Number[XXX]), but no citation. The air bags deployed. Then, the vehicle accelerated uncontrollably 260 yards and crashed into a tree where the vehicle came to a stop. A police report was filed. The contact stated his father sustained injuries and was transported out of the vehicle to the hospital by emergency units. The contact stated his father deceased 4 hours later. The vehicle was towed to a tow lot, where it was totaled. The manufacturer was made aware of the failure. The approximate failure mileage was 2,000.

How to read an unverified report.

Anyone can file one, about any vehicle, at any time. Nothing is checked before it is published, no company is asked to respond, and the person filing is not under oath. A complaint is an allegation, and a cluster of them is a signal worth investigating rather than a finding.

What makes them worth holding is that they are the only channel in this entire product that the operator does not control. Every crash report, mileage figure, disengagement count and permit filing elsewhere on this site was written by the company being described. These were written by the people it drove past.
